Reducing Health Disparities and Supporting Maternal And Child Health

Project NAF: (Nurturing Asheville and Area Families)

MZCD, Inc. has been providing services, case management, and support to pregnant and postpartum African American women and their children through Project NAF since 1998. Project NAF has been cited as a National Community Initiative Model to Eliminate Health Disparities, led by the U.S. Bureau of Primary Health Care.


We are honored to have been able to serve the community throughout this time and look forward to continuing our successful partnership. Project NAF staff members are dedicated to the provision of services and support to the community.

  

  • Since 1997, Project NAF (Nurturing Asheville and Area Families), our Infant Mortality and Maternal and Child Health program has ensured Black/African American pregnant/post-partum mothers have pregnancy related resources to enable healthy births and reverse maternal and infant mortality rates. Project NAF utilizes a research-based approach (supported by evidence-based strategies, proven effective to support mothers and their babies over a period of 2 ½ years. This program has received national recognition.
  • Project NAF: Recognized as a National Infant Mortality Reduction Model by the US Bureau of Primary Health Care. This event was hosted by MAHEC (Asheville) and sponsored by the Buncombe County Medical Society. 
  • 2011: the NCDHHS Women’s Health Division recommended Project NAF Program as a Community program model during the NC Child Fatality Task Force, and as a state model to the Appalachian Regional Commission (ARC) in Washington, DC. Project NAF was presented during the Annual ARC Conference: “Healthy Families: Healthy Futures” in September 2011. 
  • 2021: Project NAF was one of five programs chosen by Aeroflow Healthcare to receive a donation of $5,000 on Giving Tuesday.
  • 2021-Present: Project NAF was also chosen as a program for the Loaves and Fishes, an alternative gifts fair in which First Presbyterian, Central United Methodist, and Trinity Episcopal Churches of Asheville join forces to enable our members and the Asheville community to give gifts that matter during the holiday season. 
  • 2021: Project NAF was chosen to be the guest of Asheville Buncombe County Christian Ministries (ABCCM) on WLOS’s Spotlight Carolina. WLOS is Asheville’s local news station.
  • 2024: Project NAF was the recipient of the Baby Booties Team Award by the NC Perinatal Association. Recognizing a regional or local team of providers who impact women and families during the perinatal period. The North Carolina Perinatal Association (NCPA) members are a diverse perinatal community of parents and providers.

    Goals & Objectives

    Goals:   

    The goal of Project NAF is for minority babies to have a healthy first year of life and beyond. 

    To reach this goal, Project NAF provides culturally sensitive and individualized services to pregnant and post-partum women.

    Participants can enroll up to 60 days post-partum). Support services are provided until the baby’s 2nd Birthday.

     MZCD’s Project NAF program has provided case management services to Black/African American pregnant and postpartum women and their babies since 1998. Additionally, services previously provided to Latino pregnant and postpartum women were discontinued because, given the healthy birth outcomes, funding for the program could not be justified at the time.

    The overarching goal of Project NAF is to contribute to the reduction of infant mortality and low birthweight births for Black/African American women and their infants in Buncombe, Henderson, and Madison Counties North Carolina; And, Impacting Black Maternal and Child Health.

    That in NC Black/African American babies die 3x the rate of a White baby before its birthday.

    Job Description: Project NAF (Nurturing Asheville & Area Families) Outreach Worker


    Education/Experience: 

    • Bachelor’s degree in social work and one year of relevant experience. 

    • Bachelor’s degree in a Human Services Field with two years of experience.

    • Associate degree in social work with two years of experience.

    • Associate degree in human services with two years’ experience working with families. 

    • Other educational experience will be considered.

    Duties & Responsibilities:

    • Responsibilities include case management/case coordination, outreach and advocacy for African American pregnant and post-partum women, their babies, and their children; women can be enrolled in the program up to 2 months post-partum.

    • Completion of an initial assessment to obtain pertinent data for optimal service delivery.

    • Mandated certification training in the ‘Partners for a Healthy Baby Curriculum.’ 

    • Conducts home visits and provides a minimum of monthly face-to-face contact and alternate second contact with project participants.

    • Secures new participants to enroll to enhance program sustainability.

    • Outreach & Collaboration with area agencies and programs to promote the program and to secure Project NAF referrals.

    • Completes required reports /paperwork to Executive Director and funding sources.

    • Attend multi-disciplinary staff meetings with area collaborators to identify a participant’s needs and to reduce a gap or the duplication of services.

    • Accompany a participant to medical and/or other appointments to provide supportive feedback as well as to ensure optimal service delivery.

    • Travel out of county to attend mandated meetings and workshops with grantors, as well as other relevant training courses to enhance educational growth and development.

    • Hours beyond the standard workday required on occasion. 

    • Performs other related duties as directed by the Executive Director.

    Skills/Abilities: 

    • Ability to develop a supportive relationship to promote the service plan. 

    • Strong written and verbal communication skills and public speaking ability. 

    • Computer skills and ability to enter data in a database.

    • Action-oriented, self-directed, person of integrity, positive attitude and mission-driven.

    • Ability to work effectively in collaboration with diverse groups of people.

    • Ability to be flexible and to work independently.

    • Person of integrity, positive attitude, mission-driven, and self-directed.

    Knowledge: 

    • Basic principles of Social Work.

    • Social Work Ethics (emphasis during employee orientation).

    • Code of Conduct (emphasis during employee orientation).

    • Ability to complete an initial assessment & service log to obtain pertinent data for optimal service delivery.
